YOUR JOURNEY / EVERY STEP, TOGETHER
A long journey.
A team beside you.
From the first hello
to the life that comes after.
There are decisions to make, people to meet and moments of waiting. We help you navigate each part with clear guidance, connected care and room for your questions.
Let’s walk through it
ONE CONNECTED JOURNEY
Every person’s pathway is different. These stages show how support can unfold; their timing and sequence depend on your circumstances and clinical assessment. We help you understand where you are, and what comes next.
THE FIRST CONVERSATION
First, we get to know you.
Every journey starts with listening.
You may have a potential donor, a referral, or simply a lot of questions. You do not need to have everything figured out before getting in touch.
What this stage can look like
- Tell us about your situation and what you hope to understand.
- We explain how the review process works and which information may be needed.
- An initial consultation helps identify the next questions to explore.
We listen, help you gather your questions and give the next step a clear shape.
“Where do I even begin?”
With a conversation. We will take it from there, one step at a time.
DONOR EVALUATION & MATCHING
Finding the path that fits.
A suitable match takes thoughtful review.
This stage starts with you and your own potential living donor. BAO Health does not find donors for patients without one. The clinical team considers you both individually, then reviews your compatibility. Blood group, tissue typing and antibody results are among the information reviewed.
What this stage can look like
- Patient and donor information is collected for clinical assessment.
- The team reviews suitability and compatibility, and explains the options.
- Where appropriate, paired kidney exchange with other patient–donor pairs may be explored to seek a more suitable match.
We coordinate information, help explain the options and keep communication moving while the team reviews potential pathways.
“What if my donor and I are not a match?”
The team can discuss paired kidney exchange involving you and your potential donor alongside other patient–donor pairs. Matching technology supports this review; it is not a donor-finding service. Any proposed match needs clinical confirmation, and a match or timeline cannot be guaranteed.
BEFORE TRANSPLANTATION
Preparing, together.
More clarity. Fewer unknowns.
Once a possible pathway is identified, preparation becomes more detailed. The patient and donor each need evaluation, conversations about their choices and an agreed plan with the transplant team.
What this stage can look like
- Further assessments and compatibility testing help confirm the proposed plan.
- Patient and donor discuss the procedure, risks and recovery with their clinicians.
- Appointments, documents and practical arrangements are brought together.
We help organise the preparations and make room for your questions. If you are travelling, we help clarify the arrangements before you arrive.
“How will I know I am ready?”
Your clinical team decides whether it is appropriate to proceed. We help you understand the plan, what remains to be done and whom to contact along the way.
THE TRANSPLANT PERIOD
A significant moment.
A connected team.
Care for you. Communication for those beside you.
When the clinical team confirms the plan, the focus shifts to admission, surgery and the hospital stay. Patients and donors have their own care needs throughout this period.
What this stage can look like
- The hospital team explains admission and the planned sequence of care.
- The surgical and clinical teams manage the procedures and monitoring.
- Recovery and discharge planning are discussed as each person progresses.
We help keep practical arrangements and communication connected, supporting patients, donors and their families alongside the hospital team.
“Will my family know what is happening?”
We help clarify the communication plan and points of contact, with updates shared in line with patient and donor preferences and the hospital’s process.
AFTER TRANSPLANTATION
Going home.
Staying supported.
A new setting, with continuing care.
Leaving the hospital is an important transition. Both the recipient and donor need a clear understanding of their individual recovery plan, follow-up arrangements and who to contact with questions.
What this stage can look like
- The treating team explains the discharge plan and any prescribed care.
- Follow-up appointments and requested checks are coordinated.
- You discuss returning home and staying in contact, especially when travelling abroad.
We help connect the hospital plan with the next stage of your life, keeping follow-up information and communication organised.
“What happens when I leave the hospital?”
You leave with a plan from your treating team. We help you understand the next appointments and contact routes, and coordinate with your local team where appropriate.
LONG-TERM FOLLOW-UP
The life ahead.
Still, together.
Our commitment looks beyond the procedure.
The transplant journey continues as life moves forward. Ongoing follow-up, everyday questions and communication with your treating team remain part of caring for both recipient and donor.
What this stage can look like
- Follow-up is planned around the clinical team’s recommendations.
- In-person visits and appropriate remote consultations help maintain contact.
- Changes in your circumstances can be discussed with the people managing your care.
We help keep the connections in place over time, between you, your local clinicians and the transplant team.
“Can I still reach out later?”
Yes. Long-term guidance and follow-up coordination are part of our approach. You can contact us as questions arise, including when your transplantation took place elsewhere.

FOR THE LIFE BEYOND THE PROCEDURE
More than a series of steps.
A life to look forward to.
Wherever you are in your journey, we can start there.
Tell us where you areBEFORE YOU TAKE THE FIRST STEP
Do I need a donor before contacting you?
You can ask us initial questions before you have a donor. However, you will need your own potential living donor to pursue the living-donor transplant pathway we support. BAO Health does not find or provide donors for patients without one. For patients who already have a potential living donor, we can help explore paired kidney exchange with other patient–donor pairs, subject to clinical assessment.
Can I join the journey if I had my transplant somewhere else?
Yes. You can contact us even if your transplant was performed at another centre. We start by understanding your situation and discussing how guidance, follow-up and care coordination can work alongside your existing transplant team.
Is there a fixed timeline?
No single timeline fits every patient and donor. Evaluation, matching, clinical readiness and the arrangements at the treating centre all affect the pace. We help you understand the next steps without promising a transplant date or a particular outcome.
