Take the plan home.
Keep the connection.
Keep discharge records, follow-up contacts and appointment details together. Bring your questions to the team managing your care.
Clinical advice, medicines and monitoring remain individual to you. Follow the instructions provided by your treating team.
Ask what you’ll need- Clear follow-up contacts
- Coordinated appointments and requested reviews
- Continuity between the transplant team and care at home
Routine coordination is not an emergency service. For urgent medical concerns, contact your treating team or local emergency services.
Does follow-up include the donor?
Yes. Donors have their own recovery and follow-up needs. We support coordination for both people around their individual clinical plans.
Can follow-up continue after I leave Türkiye?
We help coordinate ongoing communication and, where appropriate, remote consultations and contact with your local clinicians. The treating team determines which reviews need an in-person visit.
Can I contact you if my transplant took place elsewhere?
Yes. Contact us to discuss your situation and how guidance or follow-up coordination may fit alongside your existing care team.
